M302 Documenting Eligibility Factors
Purpose: This section will assist in determining whether eligibility factors are properly documented.

POL M302: DOCUMENTING ELIGIBILITY FACTORS
1. Documentation Required For Family and Children’s Programs
Family and Children’s programs require the following documentation be included in a case file:
Signed application, if client applied via paper application.
Verification of citizenship, identity, date of birth and SSN.
Detailed case notes in WES.
Verification of income.
Verification of other factors upon request.
2. Documentation Required For Aged, Blind, Or Disabled And Medicare Savings Programs
Aged, Blind, Or Disabled programs require the following documentation be included in a case file:
Signed application, if client applied via paper application.
Verification of citizenship and identity.
Medical necessity forms, if required.
Detailed case notes in WES.
Verification of all income and resources.

Using Wyoming Birth Cert. Application Tab
1. Access Wyoming Birth Cert Application tab located within ECOM. Refer to Section M604 for instructions on accessing and using Birth Cert Application.
Requesting Birth Certificates from Vital Records
1. Print letter of request on official DFS Field Office letterhead.
2. Specify one person per letter.
3. Specify individual’s:
3A. Full name at birth.
3B. Mother’s maiden name.
3C. Father’s name.
3D. Date of birth.
3E. Place of birth.
4. Include a self-addressed, stamped envelope.
5. Address to:
Department of Health, Vital Record

Obtaining Immigration Status Documentation
1. Refer to Section M604 for information on the SAVE process and contacting US Citizenship and Immigration Services (USCIS).
